Program Change Request Form

If you would like to make a change to your child’s program, whether it be adding or dropping aftercare, elementary lunch, etc… please submit a program change request form. This change does incur a $25 processing fee for program changes (adding/dropping Early/Aftercare, etc.). No processing fees are incurred for updates of database information (permissions, family contact info, etc.)
