Required Documents for Enrollment

There are several major school and state-required documents we must collect, depending on your child’s grade level/age and special circumstances. Not every document requirement will apply to every family; check the list of required documents below for which ones you need to submit. You may also email schoolmanager@montessorischoolofcu.org if you are not sure or if you have questions. Links are provided to download forms where available.

Parent Handbook Agreement

Every family is responsible for knowing and following the School’s rules, policies, and procedures. Please read the Parent Handbook carefully and then print, sign, and submit the Parent Handbook Agreement to the office by the first day of school. You may scan and email to schoolmanager@montessorischoolofcu.org, or leave it in our outdoor dropbox.

Required Documentation for General School Records

Please submit all required documents for your child’s grade level immediately upon enrollment if possible. Final submission deadlines for each document are detailed in the yellow box below. The minimum items we need that must be submitted before the child can start school are starred (*) in each section below; additional required documents may be submitted after the start of school as specified by the deadlines in the yellow box below.

Health Examination: 
This must be a full physical examination documented on the official State of IL Certificate of Child Health Examination Form, that shows up-to-date immunizations and is dated within 1 year prior to start date. An immunization record alone or a sports physical is not sufficient.

All of the above required documents, PLUS:

Certified copy of birth certificate*
(School will make a copy and return the original to you)


Toddler Information Packet forms* (completed and signed)

Links to each of the state forms are provided above for your reference; however, your healthcare providers should be able to provide these forms for you automatically if you ask for them. You should not have to make separate appointments to get these unless your child’s last appointments with the providers are past the date requirements listed above for each document.

IMPORTANT NOTE: Due to DCFS requirements, if any of the requested documents above are missing by the time of your child’s anticipated start date, we will need to delay your child’s start date until all of the documents have been submitted.

If your toddler is transferring from a daycare facility that has their records and you would like us to request their records from the facility instead of submitting them yourself, please complete and submit an MSCU Authorization for Request of Records to the main office. Please be aware that this can delay your child’s start date if your institution is unable to send us the requested records within the timeframe of your child’s start date. Your child cannot begin attending at MSCU until we have the minimum required documents listed above. If transfer records do not contain complete documentation appropriate for the child’s grade level, the family will be required to obtain new documentation as needed.

Health Examination

TRANSFERS FROM OTHER SCHOOLS

If your kindergartener is transferring from another school, please complete and submit an MSCU Authorization for Request of Records* to the main office. Please be aware that this can delay your child’s start date if your institution is unable to send us the requested records within the timeframe of your child’s start date. Your child cannot begin attending at MSCU until we have the minimum required documents (*starred) listed above. If transfer records do not contain complete documentation appropriate for the child’s grade level, the family will be required to obtain new documentation as needed.



(ALL 1ST, 3RD, 4TH, & 5TH GRADERS)

MSCU Authorization for Request of Records* from child’s most recent school (See “Transfers from Other Schools” section below). If child has been homeschooled for a year or more, then we will need this authorization form for their previous school records and any of the following which are not included in the previous school’s records: birth certificate*, most recent physical exam, eye exam from licensed optometrist/opthalmologist, dental exam from licensed dentist, and any relevant academic/progress notes from homeschooling. If child has always been homeschooled, please provide all of the items listed.)

(Students new to Illinois schools ONLY): Eye Exam on Illinois Form completed by Oct 15, 2024

TRANSFERS FROM OTHER SCHOOLS

If your 6th grader is transferring from another school, please complete and submit an MSCU Authorization for Request of Records* to the main office. Please be aware that this can delay your child’s start date if your institution is unable to send us the requested records within the timeframe of your child’s start date. Your child cannot begin attending at MSCU until we have the minimum required documents (*starred) listed above. If transfer records do not contain complete documentation appropriate for the child’s grade level, the family will be required to obtain new documentation as needed.

IMPORTANT STATE-REQUIRED DEADLINES FOR DOCUMENTS:

Health Exams: In accordance with Illinois School Code, students must be excluded from school until the health exam form is submitted if it has not been submitted by October 15th of the upcoming school year, unless a waiver/exemption or proof of appointment within 60 days of the deadline is submitted. If the appointment goes unattended, the child must be excluded after the date of the appointment until the health exam form or a waiver/exemption has been submitted.

Eye Exams: Report cards may be withheld until the eye exam form has been submitted if it has not been submitted by October 15th of the upcoming school year, unless a waiver/exemption or proof of appointment within 60 days of the deadline is submitted.

Dental Exams: 
Report cards may be withheld until the dental exam form has been submitted if it has not been submitted by May 15th of the upcoming school year, unless a waiver/exemption or proof of appointment within 60 days of the deadline is submitted. If the appointment goes unattended, the child may not attend school in the following year until the dental exam or a waiver/exemption has been submitted.

 

If for some reason you need a health, eye, or dental waiver/exemption form, or will need to turn in your health form after August 10th, please let us know.

 

Required Documentation for Special Medical Needs

If your child has special medical needs, you MUST report them to the office and your child may not start school until the following have been provided to the school. Per Statute 105 ILCS 5/10-22.21b (2000), we do NOT administer medication during school hours/school-related activities unless absolutely necessary for the critical health and well-being of the student, and we do NOT administer OTC medication. If a child must take prescribed critical health/well-being medication during the school day (e.g., Epi-Pen, diabetes medication, seizure medication, etc.), please fill out the appropriate form to authorize the staff to administer the prescription medication.

Forms Required by Situation

Allergy Action Plan, completed and signed by a licensed healthcare provider.

– Epi-Pens with copy of prescription label provided (will be kept in the main office)

*For elementary/secondary students who wish to self-carry/self-administer epinephrine as needed, the “self-carry” and/or “self-administer” options on the Food Allergy Emergency Action Plan must be checked, and the office staff and teacher notified.

Asthma Action Plan, completed and signed by a licensed healthcare provider.
– Copies of any prescription labels containing the name of the asthma medication(s) and prescribed dosage, and the circumstances of administering the medication.
Any medications required to manage asthma during the school day (will be kept under lock in the main office)

**Elementary/secondary students who wish to self-carry/self-administer asthma medication as needed, the “self-carry” and/or “self-administer” options must notify office staff and teacher, and complete a signed Request for Self-Administration of Asthma Medication form.

Seizure Action Plan, completed and signed by a licensed healthcare provider.
– Copies of any prescription labels containing the name of the seizure medication(s) and prescribed dosage, and the circumstances of administering the medication.
Any medications required to manage seizures during the school day (will be kept under lock in the main office)

– Please inform us if your child has or develops any other significant medical needs. You may email schoolmanager@montessorischoolofcu.org to notify us if you did not originally indicate it in your enrollment application.
– Please be aware that MSCU does not have a certified nurse on staff and due to 105 ILCS 5/10-22.21b cannot administer medication unless absolutely necessary for the critical health and wellbeing of the student. If your child needs medication on a regular basis during school hours, we are happy to work with you to make arrangements to have you come and administer it to your child during the school day. Either way, it is important to note your child’s medical needs to the office so we stay informed of your child’s needs.

REMINDER: Students who have indicated that they experience medical issues that may impact their critical health and well-being (such as those listed above) may not start school until the items relevant to their medical needs have been submitted to the officePlease submit these items to the office at least 1 business day prior to your child’s start date.

Waivers

Families who decline immunizations, physical exams, dental exams, or vision exams must submit the appropriate official waiver for each State-mandated exam signed by a healthcare provider. Illinois does not allow waivers for personal or philosophical reasons.

What's the Best Way to Submit My Child's Documents?

Please bring a hard copy of all required items relevant to your child in an envelope labeled “[Child’s Name]: Required Documents” to the main MSCU office if possible. If you would like us to return your original documents to you, please include a note with your items in the envelope and we will be sure to do so.

Alternatively, you may drop your child’s documents off after-hours in the black drop box on the wall to the left of the front door of our main building off of Regency Drive East. 

To drop off emergency medications, please call the office at (217) 356-1818 to arrange for a time to drop the medications off to one of our office staff. Medications should not be dropped off in the drop box.
